Printable Forms   Select any of the following documents and forms to review, print or save a personal copy.   Downloads 2010 Football/Cheerleading Registration Information Includes dates, costs, and form requirements for registering your child in the NGFA. 2009 NGFA Handbook This handbook includes important information about the NGFA including dates, contacts, code of ethics, rules of the game, etc. 2010 GFL Physical Form All football players must have a physical on file before they can participate in any activity on the football field. Per the GFL, physicals must be signed by a MD, DO or PA. They will not accept a physical signed by a Nurse Practicioner. 2010 NGFA Sponsorship Form 2010 NGFA Background Check Form NGFA Background Check for Coaches and Volunteers. This form will need to be printed, filled out and mailed to NGFA along with a copy of your Driver's License. Mail to: NGFA, PO Box 283, Suwanee, Ga 30024 NGFA Scholar-Athlete Application Packet Application to become the next NGFA Student Athlete. One whose academic and athletic careers have been truly exemplary and whose personal standards (i.e., integrity, honesty, etc.) and accomplishments are a positive model for others. Sponsor Reimbursement Form GFL Coaches Certification Form GFL Official Roster Form (for certification & Black book) These forms are to be filled out and presented to the GFL during certification.
